Job summary

FOX Rehabilitation is seeking a licensed Physical Therapist for Wayland, MA. You will deliver one-on-one PT services to older adults in private residences or senior living communities, using our Geriatric House Calls model to improve mobility and independence.

The role emphasizes autonomy, flexible scheduling, and the ability to build lasting patient relationships. Telehealth components may be used to extend care and access for patients while maintaining high clinical standards.

Qualifications

• Licensed Physical Therapist in MA or eligible to obtain license
• PT degree from an accredited program is required
• Basic computer and EMR proficiency
• Current CPR certification

Responsibilities

• Provide one-on-one physical therapy to older adults in home or senior living settings
• Develop individualized plans of care and deliver evidence-based treatment
• Deliver therapy sessions to support mobility, independence, and safety
• Document care within FOX EMR per regulatory standards
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and staff on safety and aging in place
• Provide telehealth or virtual care when appropriate to extend access
