Comprehensive Rehab Consultants (CRC) is seeking a PM&R clinician to provide expert physiatry services at designated SNFs/post-acute centers in Freehold/Little Egg Harbor Township, NJ. This full-time role focuses on improving patient independence and functional outcomes with an autonomous daily workflow.

You will collaborate with a multidisciplinary team, lead facility-based care plans, and participate in ongoing education and leadership opportunities to advance CRC's mission and clinical

Qualifications

• Nurse Practitioner/APRN/PA license.
• New Jersey license.
• Experience in post-acute space preferred.
• Ability to work in the U.S.

Responsibilities

• Provide specialized PM&R/Physiatry services as a rehab consultant.
• Represent CRC in your facility with strong clinical leadership and collaboration with the interdisciplinary care team.
• Collaborate with PCP teams to manage SNF conditions such as CVA and postoperative surgeries.
• Work with PT, OT, and SLP teams to improve patient function and quality of life.
• Track patient progress in therapy and address barriers to therapy.
