Job summary

FOX Rehabilitation is seeking licensed Physical Therapists to deliver dedicated one-on-one care to older adults in private homes or senior living communities in Dover, DE. You will evaluate patients, design personalized care plans, and provide evidence-based therapies to improve mobility and independence.

The role offers autonomy and flexibility with the ability to create your own schedule and incorporate telehealth to extend access and continuity of care.

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living communities.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that promote mobility and independence.
• Document in EMR per regulatory standards.
• Collaborate with an interdisciplinary team.
