Job summary

Alliance for Care at Home seeks a Board-certified Nurse Practitioner to deliver hospice and palliative care in home settings. You will perform age-appropriate assessments, manage complex patients, and coordinate referrals and transitions across care settings.

The role requires independence and teamwork, strong computer skills (Word, Excel, PowerPoint), initial home evaluations and monthly visits, IDT participation, and adherence to CMS/Medicaid guidelines to ensure high-quality, patient-centered

Qualifications

• Must be Board certified.
• APRN licensure and NP certification in Hospice or Palliative Care.
• 5+ years hospice/palliative care experience; 2+ years NP role preferred.
• Independent worker with strong teamwork and communication.
• Familiarity with UM standards, CMS guidelines, and Medicaid/Medicare contracts.

Responsibilities

• Assists with palliative care in accordance with regulations.
• Orders/interprets diagnostic tests for age-specific needs.
• Implements interventions to reduce hospitalizations.
• Collaborates with care team to manage referrals and follow ups.
• Facilitates transitions between care settings (admitting, transferring, discharging).
• Prescribes appropriate treatments and monitors effectiveness.
• Performs age-appropriate history/physicals for complex patients.
• Initial home evaluation and monthly visits.
• Attends IDT and other team meetings.
