Job summary

McLeod Health is seeking an experienced Nurse Practitioner to join Palliative Care Services in a community-based setting. This role focuses on symptom management, advance care planning, and care coordination for patients with serious chronic illnesses.

The position offers a Monday–Friday schedule, no on-call duties, and collaborative support from a seasoned palliative care team, with opportunities to practice autonomously in a supportive environment.

Qualifications

• Graduate of an accredited Nurse Practitioner program.
• Board certified and/or eligible for South Carolina licensure.
• Minimum of two years of advanced practice experience required.
• Strong communication and relationship-building skills.
• Experience in hospice, palliative care, geriatrics, home health, oncology, or chronic disease management strongly preferred.

Responsibilities

• Conduct patient assessments.
• Manage complex symptoms and pain.
• Facilitate goals-of-care discussions.
• Evaluate hospice eligibility.
• Coordinate care across the healthcare continuum.
