Job summary

Comprehensive Rehab Consultants is seeking a PM&R clinician (NP/PA) to provide rehabilitative care at designated SNF/post-acute centers in Rockville/East Hartford, CT. This full-time role includes a 40-hour week with typical hours around 9 AM–4 PM.

The position starts at $130,000 per year with a robust benefits package, ongoing education support, and opportunities for clinical leadership within CRC.

Qualifications

• Experience in post-acute space preferred.
• Nurse Practitioner/APRN/PA license and Connecticut license.
• Ability to work in the U.S. and deliver patient-centered care.
• Commitment to continuous improvement and growth within CRC.

Responsibilities

• Provide specialized PM&R/Physiatry services as a rehab consultant.
• Represent CRC in your facility through leadership and collaboration with the care team.
• Collaborate with PCP teams to manage SNF conditions (CVA, neurological conditions, postoperative care).
• Work with PT/OT/SLP teams to improve function and quality of life.
• Follow therapy patients to track progress in therapy.
• Assess and manage barriers to therapy (pain, spasticity, neurogenic issues).
• Be an integral part of the SNF team by attending multidisciplinary meetings.
• Engage in culture building, Didactic lectures, regional meetings, annual dinners.
