Job summary

Baylor Scott & White Institute for Rehabilitation in Dallas, TX seeks a skilled Physical Therapist (PT) for Outpatient At Home coverage. The clinician will deliver outpatient PT in home settings across Tarrany County and the Central/South Fort Worth area, with strong professional autonomy.

Responsibilities include evaluation, treatment, case management, and patient assessment, with typical caseload 10–12 patients daily.

Qualifications

• Current state licensure or eligibility for state licensure as a Physical Therapist.
• BLS certification or completion within 90 days of employment.
• Prior outpatient and/or home health experience preferred.

Responsibilities

• Evaluation, treatment, case management, and patient assessment of orthopedic or neurological conditions.
• Maintain a patient case load of 10-12 patients per day.
• Timely, accurate documentation and data interpretation from medical notes and surgery reports.
• Ensure medical orders are transcribed and processed correctly.
• Promote continuous quality improvement and patient/family instruction as needed.
