Job summary

FOX Rehabilitation is seeking a licensed Occupational Therapist to deliver one-on-one, in-home and senior living care across Bayonne, Jersey City, and surrounding NJ communities. You will evaluate patients, create plans of care, and deliver evidence-based OT sessions in homes or communities, with flexibility to schedule visits and telehealth when appropriate.

This role offers competitive compensation ranging from $70,000 to $95,000 annually for full-time positions, plus benefits such as health

Qualifications

• Valid Occupational Therapy license in NJ or eligibility to obtain.
• EMR proficiency and documentation skills.
• Current CPR certification.

Responsibilities

• Provide one-on-one OT to older adults in homes or senior living communities.
• Perform evaluations, develop individualized plans of care, deliver evidence-based treatment.
• Deliver therapy sessions supporting wellness, mobility, independence.
• Document in FOX's EMR per regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients and caregivers to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate.
