Workable is seeking a Nurse Practitioner or Physician Assistant to provide intermittent coverage at two skilled nursing facilities in Sacramento County, CA. The role emphasizes autonomy with support from Medical Director and Medical Coordinator, clinical variety, longitudinal care, and interdisciplinary collaboration in post-acute and long-term settings.

You will see ~17 patients daily, work onsite, and coordinate with the Medical Director.

Qualifications

• Proven NP or PA in primary care or SNF settings.
• Geriatric background preferred.
• CA NP or PA license and active furnishing license.
• Current CA DEA certificate.
• Knowledge of health and patient care regulations.
• Strong ethics and excellent communication.
• Team-oriented with a positive attitude.
• Willingness to continue gaining knowledge and clinical experience.

Responsabilités

• Evaluate admissions and ensure discharge/rehab plans are executed.
• Monitor for acute changes; diagnose and treat common conditions.
• Propose treatments for chronic and infectious diseases.
• Maintain accurate records and scheduling.
• Participate in quality improvement programs.
• On-call via phone or text.
• Assist with Advanced Directives/POLST documentation.
