Job summary

Solomon Medical seeks a full-time Nurse Practitioner or Physician Assistant to support post-acute and long-term care at two skilled nursing facilities in Millbrae and Alameda, California. You will work with the Medical Director and Medical Coordinator, providing intermittent coverage for rehab patients and long-term residents while practicing in a team-oriented environment.

The role emphasizes autonomy with expert support, continuing education, and collaboration across disciplines.

Qualifications

• Proven experience as an NP or PA in primary care or sub-specialty outpatient clinics, hospitals, or SNFs.
• Geriatric background preferred but not required.
• Valid NP or PA license in California.
• Active NP furnishing license.
• Current CA DEA certificate.
• Knowledge of health and patient care regulations.
• Strong ethics & excellent communication skills.

Responsibilities

• Evaluate patients from admission and ensure discharge and rehab plans are executed.
• Monitor for acute changes; diagnose and treat common conditions.
• Propose treatments for chronic and infectious diseases.
• Maintain accurate records and schedules.
• Participate in quality improvement programs.
• Available via phone or text when on call.
• Assist with initiating/documenting Advanced Directives/POLST form.
• Daily visits: review labs, assess rehab progress, discuss concerns with patient and family.
