Job summary

Solomon Medical in California is seeking a full-time NP or PA to provide intermittent coverage at two SNFs in Sacramento County. This onsite role offers autonomy with support from the Medical Director and Medical Coordinator, clinical variety, longitudinal care, and impact in post-acute and long-term settings.

You would work 3 days in Millbrae and 2 days in Alameda, seeing about 17 patients a day, with on-call availability and a team-based approach to care while advancing your clinical education

Qualifications

• Proven experience as NP or PA in primary care, sub-specialty outpatient clinics, hospitals, or SNFs.
• Geriatric background preferred but not required.
• Valid California NP or PA license.
• Active NP furnishing license and CA DEA certificate required.
• Knowledge of health and patient care regulations; strong ethics and communication.

Responsibilities

• Evaluate patients from admission and ensure discharge and rehab plans are executed.
• Monitor for acute changes; diagnose and treat common conditions.
• Propose treatments for chronic and infectious diseases.
• Maintain accurate records and schedules.
• Participate in quality improvement programs.
• On-call via phone or text when needed.
• Assist with initiating/documenting Advanced Directives/POLST.
