Job summary

Workable is seeking a Nurse Practitioner or Physician Assistant to join a two-site SNF post-acute care practice in Monterey County, California. You will deliver direct patient care with strong physician support and coordinate care plans with an interdisciplinary team.

You will see approximately 17 patients daily, with coverage for both post-acute rehab and long-term residents. The role emphasizes autonomy within a team, ongoing education, and a focus on quality patient outcomes.

Qualifications

• Active California NP or PA license required.
• CA DEA certificate and furnishing license must be current.
• Experience in geriatric or post-acute care is preferred.

Responsibilities

• Provide intermittent coverage for post-acute rehab patients and long-term residents.
• Develop and coordinate comprehensive care plans with Medical Director and Coordinator.
• Evaluate patients at admission and ensure discharge/rehab plans are followed.
• Monitor for acute changes, diagnose and treat common conditions.
• Order tests, review results, and participate in quality improvement programs.
• Communicate with patients, families, and care teams; participate in on-call duties.
