Qualifications

• Licensed nurse practitioner (NP) or physician assistant (PA) with current state license.
• Master's degree in nursing or physician assistant studies.
• Strong leadership, interpersonal and problem-solving skills; excellent communication.

Responsibilities

• Deliver high-quality primary care to residents in skilled nursing, assisted living and independent living communities.
• Diagnose, treat and manage acute and chronic conditions with emphasis on prevention.
• Build meaningful patient relationships and coordinate care with physicians and teams.
• Maintain accurate EMR documentation and ensure regulatory compliance.
