Job summary

Huntington Health Physicians is seeking a Nurse Practitioner to deliver comprehensive services primarily in a medical office setting, with occasional hospital work. Qualifications

• MS in Nursing required.
• Minimum of 2 years’ experience as a RN or NP; outpatient setting preferred; experience with EHR preferred.
• Current RN license issued by CA BRN
• Current NP Furnishing License issued by CA BRN
• Current NPI number (CMS)
• Current DEA number
• BLS certification required
• COHN-S preferred (optional)

Responsibilities

• Coordinates and case manages the patient through the full continuum of care.
• Liaises between community physicians and Huntington Health Physicians’ resources.
• Provides clinical and professional leadership for patients, families, and staff.
