Job summary

Huntington Health Physicians is seeking a Nurse Practitioner to provide comprehensive services primarily in the medical office and occasionally in hospital settings. The NP will work with physician partners, coordinate care, and supervise care plans while delivering high-quality patient care.

Required: MSN, CA BRN RN license, NP Furnishing License, NPI, DEA, BLS. Preferred: COHN-S. Location: 65 N Madison Ave, Pasadena, CA. Full-time days shift.

Qualifications

• Master of Science in Nursing required.
• Minimum 2 years’ experience as RN or NP; outpatient setting preferred.
• Current unrestricted RN license issued by CA BRN.
• Current unrestricted NP with Furnishing License issued by CA BRN.
• Current NPI with CMS.
• Current DEA number.
• Current BLS from AHA.

Responsibilities

• Coordinate and case manage patients through the full continuum of care.
• Liaise between community physicians and Huntington Health Physicians’ resources.
• Provide clinical leadership and patient-focused care in a team setting.
