Job summary

Impact Health PC is seeking a Psychiatric Mental Health Nurse Practitioner (PMHNP) to provide psychiatric care to residents within Skilled Nursing Facilities in the Waterville, ME area. You will assess, diagnose, and manage psychiatric and behavioral health conditions, and collaborate with physicians, facility staff, and interdisciplinary teams to ensure high-quality patient care.

Qualifications

Active PMHNP license/certification in Maine.
Minimum 2 years of clinical experience providing mental health care to adults.
Experience in Skilled Nursing, Long-Term Care, Geriatrics, Hospital, Residential Care, or Outpatient Psychiatry preferred.
Proficiency with electronic health records (EHRs) and clinical documentation.
Strong communication, organizational, and collaborative skills.

Responsibilities

Provide comprehensive psychiatric evaluations and ongoing medication management.
Assess, diagnose, and treat psychiatric and behavioral health conditions.
Develop evidence-based, individualized treatment plans.
Collaborate with physicians, nursing staff, and interdisciplinary care teams.
Provide patient and family education regarding diagnoses, medications, and treatment plans.
Complete timely clinical documentation and maintain compliance with applicable regulations.
