Job summary

Impact Health PC in Augusta, ME is seeking a Nurse Practitioner to provide comprehensive care in skilled nursing facilities. You will assess and manage acute and chronic conditions, develop evidence-based treatment plans, and collaborate with physicians, facility staff, specialists, and interdisciplinary care teams.

The role offers a Monday–Friday schedule with no nights, weekends, or holidays, and competitive earnings up to $200k+ annually with robust clinical support and ongoing education

Qualifications

Active Nurse Practitioner license in Maine required.
Experience in SNF, hospitalist, ED, or ICU preferred.
Strong communication and organizational skills.

Responsibilities

Assess, diagnose, and manage acute and chronic conditions.
Develop evidence-based treatment plans.
Complete timely clinical documentation and orders.
Collaborate with physicians, nursing staff, specialists, and care teams.
Educate patients and families regarding conditions and treatment plans.
Maintain compliance with facility, state, and federal regulations.
