Job summary

Deer Oaks Behavioral Health is seeking a Licensed Professional Counselor (LPC) to join our clinical team working onsite in long-term care facilities. You will provide therapy, assessments, care planning, and behavioral health support for older adults in a supportive, mission-driven environment.

The role offers a predictable Monday–Friday schedule, autonomy, and robust administrative and clinical support to help you focus on delivering high-quality care.

Qualifications

• Strong written, verbal, and interpersonal communication skills.
• Ability to work independently while collaborating with an interdisciplinary team.
• Valid driver’s license and reliable transportation.
• Active, unrestricted LPC license.

Responsibilities

• Provide individual, family, and group therapy to LTC residents.
• Conduct clinical assessments, psychosocial evaluations, and treatment planning.
• Collaborate with nursing, social services, rehabilitation, and medical providers to coordinate care.
• Maintain complete and timely documentation using the GNT and ensure HIPAA compliance.
