Job summary

Deer Oaks Behavioral Health seeks a Licensed Professional Counselor to join our integrated clinical team onsite at local long-term care facilities. You will provide therapy, assessments, care planning, and behavioral health support to older adults in LTC settings.

The role offers autonomy with a predictable Monday–Friday schedule and robust administrative and clinical support so you can focus on delivering high-quality care.

Qualifications

• Active, unrestricted LPC license.
• Valid driver’s license and reliable transportation.
• Strong written, verbal, and interpersonal communication skills.
• Ability to work independently while collaborating with an interdisciplinary team.
• Commitment to timely documentation and high‑quality care.

Responsibilities

• Provide individual, family, and group therapy to LTC residents.
• Conduct clinical assessments, psychosocial evaluations, screenings, and treatment planning.
• Support residents with depression, anxiety, adjustment to illness, grief, trauma, substance use, and behavioral disturbances.
• Deliver behavioral management and brief, goal‑oriented interventions.
• Work with nursing, social services, rehabilitation, and medical providers to coordinate treatment.
• Participate in care plan discussions and facility meetings.
• Maintain complete and timely documentation using the GNT.
• Follow ethical, professional, and regulatory guidelines.
