Job summary

Deer Oaks Behavioral Health — Long-Term Care Clinical Services is seeking a Licensed Professional Counselor (LPC) to join our integrated clinical team. The LPC will provide therapy onsite in one or more long-term care facilities within the local community, focusing on geriatric behavioral health and chronic medical conditions.

The role emphasizes autonomy within a collaborative interdisciplinary setting, with responsibilities including assessments, care planning, and documentation using the GNT,

Qualifications

• Active, unrestricted LPC license.
• Valid driver’s license and reliable transportation.
• Ability to work independently while collaborating with an interdisciplinary team.
• Timely documentation and high-quality care.

Responsibilities

• Provide individual, family, and group therapy to LTC residents.
• Conduct clinical assessments, psychosocial evaluations, screenings, and treatment planning.
• Support residents with depression, anxiety, adjustment to illness, grief, trauma, substance use, behavioral disturbances in LTC.
• Deliver behavioral management and brief, goal-oriented interventions aligned with geriatric care.
• Work with nursing, social services, rehabilitation, and medical staff to coordinate treatment.
• Participate in care plan discussions, facility meetings, and case collaborations.
