Job summary

FOX Rehabilitation is seeking a licensed Speech Language Pathologist to deliver one-on-one care to older adults in Wilmington Island, GA, home-based or within senior living communities. You’ll evaluate, plan, and provide evidence-based therapy, using our Geriatric House Calls model and optional telehealth to extend access.

You’ll work independently with autonomy and a flexible schedule, collaborating with an interdisciplinary team and documenting in FOX’s EMR.

Qualifications

• Valid Speech Language Pathology license in the state(s) of practice.
• Degree from an accredited Speech Language Pathology program.
• Current CPR certification.
• Current Certificate of Clinical Competence (ASHA CCC).

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults in homes or senior living communities.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to support patient wellness, mobility, and independence.
• Document in FOX’s EMR system in accordance with regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.
