Job summary

FOX Rehabilitation is seeking a licensed Speech Language Pathologist to provide in-home and senior living care in Winchester, VA. You’ll conduct evaluations, create individualized care plans, and deliver evidence-based therapy in patients’ homes or community settings.

The role emphasizes autonomy, flexible scheduling, and collaboration with an interdisciplinary team, with optional telehealth components to expand access and continuity of care.

Qualifications

• Valid Speech Language Pathology license in the state(s) of practice.
• Degree from an accredited Speech Language Pathology program.
• Basic computer and EMR proficiency.
• Current CPR certification.
• Current CCC credential (ASHA) is preferred.

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults in homes or senior living communities.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, longevity, mobility, and independence.
• Document effectively within FOX’s EMR system per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access and flexibility.
