Job summary

FOX Rehabilitation is seeking a licensed Speech Language Pathologist to provide one-on-one care to older adults in New Haven, CT, including private residences and senior living settings.

Clinicians will conduct evaluations, create individualized plans, and deliver evidence-based treatment while documenting in FOX’s EMR system. Telehealth integration and flexible scheduling empower you to balance patient care with your personal availability.

Qualifications

• Must hold a valid Speech Language Pathology license in the state of practice or be eligible to obtain one.
• Degree from an accredited Speech Language Pathology program.
• Current CPR certification.
• Current Certificate of Clinical Competence (ASHA CCC).

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults in private residences or senior living communities.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence.
• Document effectively within FOX’s EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.
