Job summary

FOX Rehabilitation in West Hartford, CT seeks a licensed Speech Language Pathologist to deliver one-on-one care in homes or senior living communities. The role offers autonomy, flexible scheduling, and the opportunity to impact longevity and independence through our Geriatric House Calls model.

You will evaluate, plan, and provide evidence-based therapy, document in our EMR, and collaborate with an interdisciplinary team. Remote telehealth options may be available.

Qualifications

• Licensed Speech-Language Pathologist in CT.
• ASHA CCC is required.
• CPR certification current.

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults in homes or senior living communities.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, longevity, mobility, and functional independence.
• Document effectively within FOX’s EMR system per regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth / virtual care when appropriate to extend access.
