Job summary

FOX Rehabilitation in Ridgefield, CT is seeking licensed Speech Language Pathologists to deliver one-on-one care in patients' homes or senior living communities. You will assess, plan, and provide evidence-based therapy to improve communication, safety, and independence.

The role offers autonomy, flexible scheduling, and potential telehealth options to extend reach. Candidates should hold a valid SLP license in CT (or eligible), CCC, CPR, and be comfortable traveling within CT.

Qualifications

• Valid State license to practice Speech-Language Pathology
• Current CCC from ASHA or eligible to obtain
• Basic computer and EMR proficiency
• Current CPR certification
• Ability to work independently with patients in home or community settings

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults in home or senior living settings
• Perform evaluations and develop individualized plans of care
• Deliver therapy sessions to improve wellness, mobility, and independence
• Document in FOX’s EMR system following regulatory standards
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and staff to promote aging in place
• Provide telehealth/virtual care when appropriate
