Job summary
Fox Rehabilitation is seeking a licensed Speech Language Pathologist to deliver one-on-one care to older adults in private residences and senior living communities in Manchester, CT. You’ll work within our Geriatric House Calls™ model, maintaining autonomy and flexibility to manage your treatment schedule and patient relationships.
The role supports in-home and community-based care, with optional telehealth to extend access.
Qualifications
- Licensed Speech Language Pathologist in CT or eligible to obtain
- Current CPR certification
- CCCs with ASHA
- Strong communication and interpersonal skills
Responsibilities
- Provide one-on-one Speech Language Pathology to older adults in homes and senior living communities
- Perform evaluations and develop individualized plans of care
- Deliver therapy sessions to improve wellness, mobility and independence
- Document in FOX EMR per regulatory standards
- Collaborate with PTs, OTs, SLPs and Fitness Specialists
- Educate patients, caregivers and staff to promote aging in place
- Provide telehealth / virtual care when appropriate
