Job summary

FOX Rehabilitation in Jamaica, NY is seeking licensed Physical Therapists to deliver one-on-one care to older adults in private homes or within Senior Living Communities. You will evaluate, plan care, and provide evidence-based therapy, while building lasting relationships and managing your own schedule.

Qualifications

• Valid Physical Therapy license in the state of practice or eligibility to obtain
• Degree from an accredited Physical Therapy program
• Basic computer and EMR proficiency
• Current CPR certification

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living communities
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
• Deliver therapy sessions to promote mobility, independence and function
• Document in FOX’s EMR system per regulatory standards
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists)
• Educate patients, caregivers, and staff to promote safety and aging in place
• Provide telehealth/virtual care when appropriate to extend access
