FOX Rehabilitation is expanding its Physical Therapy team in Oswego, IL, delivering one‑on‑one care to older adults in homes and senior living communities. Therapists create and manage their own schedules while building strong patient relationships.

The role may include telehealth components for flexible care delivery. Ideal candidates are licensed PTs with EMR proficiency, CPR certification, and a degree from an accredited PT program.

Qualifikationen

• Active Physical Therapy license in state(s) of practice or eligibility.
• Degree from an accredited Physical Therapy program.
• Proficiency with EMR systems and basic computer tasks.
• Current CPR certification is required.

Aufgaben

• Provide one-on-one physical therapy to older adults in homes or senior living settings.
• Perform evaluations, develop individualized care plans, and deliver evidence-based treatment.
• Deliver sessions that support mobility, independence, and function.
• Document thoroughly in FOX’s EMR per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and fitness specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Offer telehealth/virtual care when appropriate to extend access.
