Job summary

FOX Rehabilitation is hiring a licensed Physical Therapist to join our Jamesburg, NJ team, delivering in-home and senior living care to older adults. You’ll conduct evaluations, develop individualized plans, and provide one-on-one therapy sessions in private homes or communities, with the flexibility to manage your own schedule and leverage telehealth when appropriate.

Qualifications

• Licensed Physical Therapist in NJ or eligible to obtain license.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one PT to older adults in homes or senior living communities.
• Perform evaluations, develop individualized plans of care, deliver evidence-based treatment.
• Deliver therapy sessions that promote mobility and independence.
• Document in FOX’s EMR system per regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.
