Job summary

FOX Rehabilitation is expanding in Johns Creek, GA, delivering one-on-one physical therapy to older adults in private residences and senior living communities. Therapists control their schedule, may include telehealth, and join a clinician-driven team focused on meaningful, functional outcomes.

Positions emphasize autonomy, flexibility, and strong patient relationships within the Geriatric House Calls model. In-home services foster independence and aging in place.

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Current CPR certification.
• Basic computer and EMR proficiency.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support mobility, independence, and function.
• Document effectively within FOX’s EMR system in accordance with regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
