Job summary

TheraCare is seeking a licensed Occupational Therapist (OT) for a Full-Time home health opportunity providing in-home therapy services to adult and geriatric patients throughout Beaumont and surrounding areas.

This Full-Time home health OT position offers flexibility, autonomy, and the ability to provide one-on-one patient care in the home setting. Clinicians can create their own schedules while helping patients improve mobility, safety, and independence in their homes.

Qualifications

• Strong communication, clinical judgment, and time-management skills.
• Experience in home health settings is preferred.
• CPR certification is required or current.

Responsibilities

• Perform home health occupational therapy evaluations and re-evaluations for adult and geriatric patients.
• Develop and update individualized POC.
• Treat patients with orthopedic, neurological, cardiac, post-surgical, or other medical conditions.
• Address ADLs, IADLs, coordination, strength, balance, and functional mobility.
• Apply manual therapy techniques and non-complex modalities as ordered.
• Implement interventions for fall prevention, home safety, and environmental modifications.
• Train patients and caregivers in adaptive equipment and DME.
• Fabricate splints and recommend assistive devices as appropriate.
• Complete accurate documentation in the EMR and communicate with physicians and care teams.
• Supervise COTAs and home health aides as applicable.
• Follow home health regulations, infection control, and safety standards.
