Job summary

TheraCare Home Health in Beaumont, TX seeks a licensed Occupational Therapist (OT) for full-time home health caseloads serving adults and geriatric patients. You will deliver one-on-one in-home OT, setting your own schedule while helping patients improve mobility, safety, and independence in familiar surroundings.

Qualifications

• Doctorate in OT from an accredited program.
• Active OT license in Texas or eligible Compact licensure.
• Home health experience (1 year) required.
• HomeCareHomeBase (HCHB) experience preferred.
• Current CPR certification.
• Strong communication, clinical judgment, and time-management skills.
• Knowledge of home health regulations and Medicare guidelines preferred.
• Valid driver's license, reliable transportation, and current auto insurance.

Responsibilities

• Perform home health occupational therapy evaluations and re-evaluations for adults and geriatric patients.
• Develop and update individualized Plans of Care (POC).
• Treat patients with orthopedic, neurological, cardiac, post-surgical, or other medical conditions.
• Address ADLs, IADLs, coordination, strength, balance, and functional mobility.
• Apply manual therapy techniques and non-complex modalities, including heat, cold, electrical stimulation, and other physician-ordered treatments.
• Implement interventions for fall prevention, home safety, and environmental modifications.
• Train patients and caregivers in adaptive equipment and durable medical equipment (DME).
• Fabricate splints and recommend assistive devices as appropriate.
• Complete accurate and timely documentation in the EMR.
• Communicate with physicians and interdisciplinary care teams.
• Supervise COTAs and home health aides, as applicable.
• Follow all home health regulations, infection control, and safety standards.
