Qualifications

• Active NP license required, with most specialties accepted except Pediatrics, PMHNP, and Women's Health.
• Must have reliable personal transportation to travel across multiple counties.
• Willingness to start work immediately is required.

Responsibilities

• Conduct in-home HRAs (medical history, lifestyle, basic evaluations).
• Provide preventive care education.
• Document visits using a company-provided iPad.
