Job summary

Source One Management Services, LLC seeks Nurse Practitioners for in-home Health Risk Assessments. This PRN/1099 contractor role requires an active NP license and travel to Cuyahoga and Lorain Counties, with flexible scheduling (50+ hours/month).

No prescribing or follow-up care is required; onboarding is fast, with training paperwork completed within 10–12 days and field work starting soon after.

Qualifications

• Active NP license; no PAs and excluding PMHNP, Women’s Health and Pediatric NP’s.
• Ability to travel locally across counties and complete onboarding promptly.

Responsibilities

• Conduct in-home Health Risk Assessments (HRAs) for Medicare Advantage members.
• Document visits using an iPad and provide preventive care education.
• Complete credentialing and onboarding within 10–12 days; start field-work promptly.
