Job summary

Source One Management Services, LLC is seeking Nurse Practitioners or Physician Assistants for flexible in-home Health Risk Assessments in Franklin, IN. This PRN role offers a self-directed schedule with an 8-day monthly minimum, competitive pay with incentives, and no prescribing or on-call duties.

Documentation is streamlined via a company-provided iPad. Qualified providers hold an active NP or PA license (excluding pediatrics and women’s health).

Qualifications

• Must hold an active NP or PA license (non-pediatrics, non-women’s health).
• Willingness to travel within roughly 60 minutes of home.
• Minimum availability of eight 7.5-hour days per month.
• 1099 contractor status with mileage considerations and simple documentation.

Responsibilities

• Complete comprehensive in-home HRAs for Medicare Advantage patients.
• Review patient history, medications, lifestyle, and social factors.
• Conduct basic physical and mental health evaluations.
• Travel locally to patients in your county and nearby areas.
