Job summary

Atlantic Visiting Nurse is seeking a compassionate Hospice Registered Nurse to join our interdisciplinary team in Morristown, NJ. You will provide expert home-based clinical care, manage an individual caseload, and collaborate with physicians, social workers, and therapists to ensure dignified end-of-life care.

Responsibilities include comprehensive assessments, care-plan development, symptom management, and education for patients and families.

Qualifications

• Current New Jersey Registered Nurse (RN) License.
• Minimum one year of nursing experience in Acute Care, Subacute Care, Home Health or Hospice.
• Experience using an Electronic Medical Record (EMR).
• Strong clinical assessment and critical thinking skills.
• Excellent verbal and written communication skills.
• Ability to work independently while managing a patient caseload.

Responsibilities

• Perform comprehensive nursing assessments to evaluate patients’ needs.
• Determine patient appropriateness for hospice services and develop care plans.
• Monitor patient progress and revise plans based on clinical changes.
• Provide symptom management and safe medication practices.
• Coordinate care with physicians, social workers, and the IDT.
• Educate patients and families about disease progression and hospice philosophy.
• Complete timely clinical documentation in the EMR and regulatory records.
