Always Best Care Senior Services – Solano & Napa Counties in Vacaville, CA is seeking a compassionate Per Diem Home Health Registered Nurse to join our growing home health team. The RN provides professional skilled nursing services in the home and collaborates with physicians, families, and the interdisciplinary team to deliver high-quality, patient-centered care.

This role offers autonomy and meaningful one-on-one patient care with flexible per diem scheduling based on patient needs.

Qualifications

• Graduate of an approved professional nursing program.
• Active California RN license in good standing.
• Minimum of 1 year of professional nursing experience.
• Home health experience strongly preferred.
• OASIS experience preferred.
• Current CPR certification.
• Valid California driver’s license.
• Current auto insurance and reliable transportation.
• Ability to pass criminal background check, DMV check, and onboarding requirements.
• Strong clinical judgment and communication skills.

Responsibilities

• Provide skilled nursing services within the scope of nursing practice.
• Complete comprehensive patient assessments, including medical history and home safety review.
• Perform assessment visits and timely documentation (OASIS and nursing notes).
• Develop, implement, and revise Plans of Care with physician and team.
• Identify needs and determine if additional home health services are needed.
• Re-evaluate nursing needs and ADL/iADL abilities.
• Supervise Home Health Aide services per state/federal requirements.
• Educate patients and families on disease management and self-care.
• Communicate status changes to physicians and care team.
• Coordinate patient services with care team and payers.
• Participate in case conferencing and quality improvement.
• Review billing concerns with patients/families when applicable.
• Maintain licensure and comply with regulations.
