Always Best Care Senior Services – Solano & Napa Counties is seeking a compassionate Per Diem Home Health Registered Nurse to join our growing home health team in Vacaville, CA. The RN will deliver skilled nursing care in patients' homes under the supervision of the Director of Patient Care Services.

This role emphasizes autonomy, meaningful one-on-one patient care, and collaboration with physicians and interdisciplinary teams to advance our expanding home health division.

Qualificações

• Graduate of an approved professional nursing program.
• Active California RN license in good standing.
• Minimum of 1 year of professional nursing experience.
• Home health experience preferred; OASIS experience preferred.

Responsabilidades

• Provide skilled nursing services requiring substantial and specialized nursing knowledge.
• Complete comprehensive patient assessments, including medical history and home safety review; develop plans of care in collaboration with physician and team.
• Perform assessment visits and complete timely documentation, including OASIS and nursing notes.
• Develop, implement, and revise individualized Plans of Care with physician and interdisciplinary team.
• Coordinate patient services with nursing, therapy, social work, home health aide services, physicians, payers, and community resources.
