Job summary

Caretenders, part of LHC Group, is seeking an RN in Home Health to provide nursing care in patients' homes as prescribed by physicians. You will coordinate the total plan of care with other professionals and communicate with families to ensure continuity of care.

Your responsibilities include developing and updating the plan of care, supervising home health aides, completing OASIS assessments, and ensuring compliance with laws and agency policies.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License and reliable transportation.
• Strong communication and organizational skills.

Responsibilities

• Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals.
• Supervises home health aides and licensed nurses and assigns tasks per regulations.
• Performs supervisory visits and ensures high quality clinical services.
• Completes assessments (OASIS), reconciles medications, and documents visits per policy.
