Job summary

Elara Caring is seeking a Home Health Registered Nurse in Chicopee/Springfield territory. You will provide one-on-one care at patients’ homes, manage caseloads, and coordinate with clinicians to support independence and safety.

Qualifications

• Current, unrestricted RN license.
• Associate degree in Nursing required; bachelor’s degree preferred.
• At least one year of clinical nursing experience.
• Willingness to travel throughout the assigned territory.
• Previous home health or hospice experience preferred.
• Homecare Homebase experience preferred.
• Dependable vehicle, valid driver’s license, and current auto insurance.
• Ability to lift 50–100 lbs.

Responsibilities

• Manage an assigned caseload of home health patients.
• Complete comprehensive assessments and ongoing evaluations.
• Develop, implement, and update individualized plans of care.
• Coordinate services with physicians, therapists, caregivers, and clinical leadership.
• Educate patients and families on medications, treatments, disease management, and safety.
• Monitor changes in each patient’s condition and communicate significant findings.
• Maintain accurate, timely clinical documentation.
• Help patients achieve better outcomes while remaining safely at home.

What You’ll Do
