Job summary

MyCHN Home Health Services is seeking a Registered Nurse (RN) to provide skilled nursing care in patients' homes across Brazoria, Galveston, and Harris Counties. The role requires collaboration with physicians, therapists, and care team members to deliver high-quality, patient-centered care.

You will perform assessments, develop care plans, administer treatments, and educate patients and families. A Texas RN license and CPR/BLS certification are required or preferred.

Qualifications

• Graduate of an accredited nursing program.
• Current Texas RN license in good standing.
• Minimum one year of nursing experience.
• Home health experience preferred.
• CPR/BLS certification preferred.

Responsibilities

• Conduct comprehensive patient assessments and evaluations.
• Develop and revise plans of care in collaboration with physicians and team.
• Provide skilled nursing services including wound care, IV therapy, and medications as ordered.
• Monitor patient status and adjust care plans as needed.
• Educate patients and families on diagnoses, treatments, and safety.
