Job summary

MedCadre is seeking an RN to provide home-based hospice/home health care to adults and children, assessing, planning, implementing, and evaluating care in the patient’s home.

Responsibilities include autonomous home visits, coordination with the health team, and ensuring compliance with standards and documentation. Strong clinical skills, communication, and ability to work in diverse home environments are essential.

Qualifications

• Ability to communicate effectively both verbally and in writing.
• Basic computer skills.
• Ability to commute to the office at least twice daily to electronically transfer patient files.

Responsibilities

• Provide clinical care in home hospice/home health setting.
• Follow nursing process and adhere to policies and regulatory standards.
• Coordinate care with interdisciplinary team and educate patients/families.
