Job summary

Home Caregivers Partnership is seeking a licensed Physical Therapist Assistant to serve home health care patients in the Loveland/Longmont area. The PTA will work under the supervision of the Physical Therapist and Clinical Manager, providing intermittent therapy in patients' homes.

Full-time and part-time opportunities are available. The role requires licensure, strong communication, and the ability to work autonomously with clinical support available as needed.

Qualifications

• Licensed, registered, or certified as a PTA in the practicing state.
• Two years experience preferred.
• Ability to exercise initiative and independent judgment.
• Acceptance of philosophy and goals of Agency.

Responsibilities

• Performs services planned, delegated and supervised by the Physical Therapist.
• Participates in in-service programs and presents in-service programs as assigned.
• Follows established Agency policies and procedures.
• Provides physical therapy per physician plan of care.
• Coordinates with PT and IDT members for patient care planning.
• Maintains patient records and progress notes in a timely manner.
