WellTech Partners seeks a Registered Physical Therapist to join our Seattle home health team. This full-time role delivers physician-prescribed PT in patients’ homes, emphasizing autonomy and meaningful one-on-one care in King County.

The ideal candidate has a CAPTE-approved PT program, WA license, and 2+ years of PT experience, with strong communication and organizational skills to manage OAISIS documentation and care plans.

Competenze

• Graduate of a PT program approved by CAPTE, APTA, or equivalent accrediting body.
• Current Washington State PT license in good standing.
• Minimum 2 years of experience as a Physical Therapist, preferably in home health.
• OASIS documentation experience preferred.

Mansioni

• Conduct initial assessments, develop plans of care, and reassess every 30 days.
• Provide skilled, physician-prescribed PT in patient homes.
• Prepare progress notes and documentation within defined timeframes.
• Instruct patients and families in home programs and activities of daily living.
• Supervise PTAs and HHAs as applicable.
• Participate in care conferences and quality improvement activities.
