Job summary

Residential Home Health and Hospice in Schaumburg is seeking an Occupational Therapist to deliver skilled home health care to patients in their homes within the Schaumburg area. The role emphasizes autonomy, meaningful patient relationships, and opportunities for career growth, with a $10,000 first-year bonus for achieving key milestones.

You will conduct ADL-focused evaluations, develop individualized treatment plans, educate patients and caregivers, and document in the EMR.

Qualifications

• Active Occupational Therapist license in the state you work.
• At least 1 year of OT clinical experience.
• Valid driver’s license and ability to drive ~20% of workday.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting in collaboration with an interdisciplinary team.
• Perform comprehensive ADL-focused evaluations and develop individualized treatment plans.
• Educate and empower patients and caregivers on adaptive techniques and home safety strategies.
• Ensure thorough, compliant EMR documentation.
• Travel to patient homes within assigned territory.
• Work independently, but collaborate with team as needed.
