Job summary

Residential Home Health and Hospice in the Westchester area is hiring an Occupational Therapist (OT) for a home health position delivering skilled, patient-centered care in patients' homes. Travel to assigned territory is required, with a focus on ADLs, safety, and independence.

Qualifications

• Active Occupational Therapist license in the state you work.
• One year of Occupational Therapist experience delivering quality patient care.
• Valid driver's license and ability to drive approximately 20% of workday.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team.
• Perform comprehensive evaluations focused on ADLs, upper extremity function, and home safety.
• Develop individualized treatment plans to promote independence and quality of life.
• Educate patients and caregivers on adaptive techniques, assistive equipment, energy conservation, and home safety strategies.
• Think critically, solve problems in real time, and ensure compliant EMR documentation.
• Travel to patient homes within assigned territory.
