Job summary

Residential Home Health and Hospice is seeking an Occupational Therapist to provide skilled home health care in the Media, PA area. This role offers autonomy, meaningful patient relationships, and career growth, with a $10,000 first-year bonus based on achieving key milestones.

You will assess ADLs, safety, and independence, educate patients and caregivers, and document thoroughly in the EMR. A valid driver’s license is required for travel to patient homes across the territory.

Qualifications

• Active OT license in the state you work
• One year of experience as an Occupational Therapist
• Valid driver’s license for 20% travel to patient homes

Responsibilities

• Deliver skilled, patient-centered care in a home health setting with an interdisciplinary team
• Perform comprehensive evaluations focusing on ADLs, upper extremity function, and home safety
• Educate patients and caregivers on adaptive techniques, assistive devices, energy conservation, and safety
• Ensure compliant EMR documentation and thorough care planning
• Work independently while staying connected with the team
• Travel to patient homes within assigned territory
