Job summary

Banner Home Health Tucson is seeking a Registered Nurse for a per diem home health role in the Tucson area. The RN will assess, plan, implement and evaluate patient care in the home environment, coordinating with a multidisciplinary team and documenting care in the EMR.

The position offers flexibility with scheduling, mileage reimbursement, and a company cell phone and tablet for documentation. BLS and an active RN license are required, with acute care experience preferred.

Qualifications

• Must possess a current, valid RN license in state of practice.
• BLS certification required.
• Experience in an acute care setting preferred.

Responsibilities

• Assess patient needs and develop an individualized plan of care.
• Implement care plans and supervise nursing activities.
• Coordinate with the health care team and communicate patient status.
• Document assessments and interventions in the patient record.
• Provide education and support to patients and families.
• Participate in quality improvement and evidence-based practice.
• Support continuity of care and discharge planning.
• Collaborate with physicians and other providers to ensure optimal outcomes.
