You will work under the supervision of a Registered Nurse and physician, delivering skilled nursing services with autonomy in a supportive environment. The role emphasizes accurate documentation in HCHB, effective communication with the care team, caregiver education, and adherence to all regulatory requirements.

Qualifikationen

• Active Michigan LPN license in good standing.
• Graduate of an accredited practical nursing program.
• Current BLS/CPR Certification.
• Valid driver's license, reliable transportation, and current auto insurance.
• Strong clinical, communication, and documentation skills.
• Ability to work independently in a home health setting.

Aufgaben

• Provide skilled nursing care in patients' homes under the direction of the Registered Nurse and physician.
• Monitor patient condition, vital signs, and overall health status.
• Administer medications and treatments as ordered and within the LPN scope of practice.
• Accurately document all visits and care provided in Homecare Homebase (HCHB).
• Communicate changes in patient condition to the supervising RN and physician.
• Educate patients and families on medications, disease management, and the plan of care.
• Ensure compliance with all federal, state, and agency regulations.
• Collaborate with the interdisciplinary team to achieve optimal patient outcomes.
