Job summary

Mass General Brigham Home Care, Inc. is seeking a Physical Therapist to deliver one-on-one care in patients' homes across East Boston, Winthrop, Chelsea, and Revere. You will evaluate mobility, safety, and independence, and develop individualized plans with clear goals.

The role offers full-time hours, Monday–Friday, with dedicated time for documentation, chart review, and coordination. Travel throughout the service area is required, backed by a supportive, interdisciplinary team.

Qualifications

• At least one year of experience in acute care, rehab, or home care.
• Current Massachusetts PT license/registration.
• Strong communication and tech skills.

Responsibilities

• Evaluate patients and develop individualized care plans.
• Provide PT focused on mobility, strength, balance and function.
• Educate patients and caregivers on home exercise and safety.
• Track progress and adjust care as needed.
• Collaborate with the interdisciplinary care team.
